TITLE: Supported Housing Specialist/Homeless Case Manager

AGENCY: The mission of Rubicon Programs Inc. is to create and deliver integrated solutions to profound social problems. Our purpose is to make a positive and lasting impact on people living in poverty and on people living with disabilities, especially psychiatric disabilities.  We empower people to move out of poverty and improve their quality of life.  Rubicon’s programs include workforce services, behavioral healthcare, case management, affordable housing, and legal services.

ACCOUNTABILITY: This position reports to the Director of Case Management Services and the Team Leader of Bridges To Home to provide case management and other supportive services to low-income, homeless and disabled Rubicon clients.

QUALIFICATIONS:
  • Two years of progressively responsible case management work experience serving individuals with major mental illnesses, homelessness, and/or substance abuse required; must be able to demonstrate working knowledge of substance abuse issues and treatment modalities. AA in social services or related field preferred; familiarity with housing related issues and Medi-Cal billing highly desired;
  • Interpersonal flexibility, solid teamwork, excellent communication skills and the ability to work successfully with minimal supervision;
  • Computer and internet literacy including Microsoft Office proficiency;
  • Proof of a valid and current driver license, current auto insurance, and an acceptable Motor Vehicle Report are required. 
  • Bilingual, English/Spanish proficiency preferred.

RESPONSIBILITIES:
  • Intensive case management including but not limited to: intake, eligibility determination, counseling, goal setting and service plans, monitoring, advocacy, referrals and follow up support; involves transporting clients to and from program sites on a regular basis;*
  • Primary and central point of contact for two co-located Rubicon housing sites; includes conducting wellness checks, facilitating tenant meetings; monitoring program compliance; providing substance abuse counseling and treatment referrals, and coordinating  with other service providers;*
  • Documentation and reporting: Includes data capture and entry at all points of the case management process  and accurate and timely reporting of same per agency standards;*
  • Participate in staff, inter-agency or other coordinating meetings; and perform other duties as required.

HOURS: This is a full-time, non-exempt position.

HIRING RANGE:  $18.20/hour - $20.05/hour depending upon skills and experience.  An additional 5% of salary may be granted for proven English/Spanish bilingual proficiency.
